Communicate with your employer: Although discussing breastfeeding may be uncomfortable at times, you should communicate your situation and your needs openly. Explain that breastfeeding requires you to nurse and/or pump throughout the work day. Explain how much time you'll need for breaks and offer to make up the time. Discuss the benefits: Talk about some of the health benefits of breastfeeding for you and your baby, including the possibility of reduced absenteeism and lower health care costs due to the fact that breastfed babies have fewer illnesses. Consider purchasing or renting a double breast pump: These pumps reduce pumping time virtually by half, so they offer you and your employer timesaving benefits. Begin pumping at home three to four weeks prior to returning to work: This will familiarize you with pumping and help to keep your milk supply up. Try to pump when the baby would normally breastfeed. Develop a schedule: Once you've established a pumping and feeding routine at home, create a schedule for work. This will vary depending on the age of your baby and your work schedule (part time, full time, phased-in return, etc.). Women whose infants are younger than three months typically need to pump every two to three hours; women with older babies typically only need to pump every three to four hours. Be accommodating: Try to arrange pump times to coincide with established breaks and lunch periods. Coordinate with your caregiver: Give your caregiver a copy of your breastfeeding schedule and written instructions on how to store and use breast milk. If you plan to breastfeed rather than pump during your lunch hour, or at other times during the day, designate a time and place to meet the caregiver and your baby. Explain that your baby should not be fed within a couple of hours of your scheduled visits. If the baby is hungry before then, the caregiver can tide the baby over with water or a snack-sized portion of stored milk.

"Transitioning back to the workplace while continuing to breastfeed can be a tremendous challenge for new mothers," said LifeCare CEO, Peter G. Burki. Summertime brings outside games, yard work, camping, swimming, trips to the beach, and long bike rides beneath the summer sun. Health care providers at the VA Maryland Health Care System (http://www. maryland. va. gov/) say staying healthy in summer requires more than eating the right foods. “In the heat of all the fun, it's important to pay close attention to your body and to the environment to avoid hidden dangers,” says Dr. Mark Olszyk, deputy chief of staff for the VA Maryland Health Care System (http://www. maryland. va. gov/). “Staying hydrated maintains the fluid balance in our bodies,” says Valerie Adegunleye, clinical nutrition manager at the health care system. Also, those with asthma and other respiratory problems need to pay attention to air quality warnings, carry their medications and keep their inhalers on hand. Below is a partial list of summer health tips to stay healthy and cool during the long dog days of summer.

1. Drink plenty of water to avoid dehydration:
Average adults lose more than ten cups of fluids daily, through sweat, breathing, and eliminating waste in addition to electrolytes that maintain the fluid balance in our bodies-- so it's up to the individual to restore them!

How do I know if I'm dehydrated?
 Thirst: Did you know that when you’re thirsty, your body is already dehydrated?  Urine color: The color of your urine tends to be the best gauge of your hydration status. A clear to light yellow color usually indicates that you're well hydrated, while a dark yellow or amber color is a good predictor of dehydration.  Dry or sticky mouth  Headache  Dizziness/lightheadedness  Weakness  Muscle cramps  Feeling of tiredness

Note: Feeling any of the above symptoms means you're already dehydrated, so don't wait for the signs to hydrate! Experiencing severe dehydration signs such as vomiting, low blood pressure, sunken eyes, fever, extreme thirst, little or no urination or lack of sweating may require medical assistance.

What should I hydrate with?

 Water: The beverage of choice if you are outside sweating for less than 60 minutes.  Sports Drinks: If you are sweating for more than 60 minutes you may require a sports drink to replenish electrolytes. However, check with your doctor to see if this is appropriate for you, as some electrolytes can be detrimental to people with certain medical conditions.  Food: Believe it or not, establishing a healthy diet that includes fruits and vegetables that have a high fluid content also contributes to about 20% of your body’s fluids (the remaining 80% is from beverages). This may include lettuce, watermelon, broccoli, grapefruit and apples, just to name a few.

What should I avoid?

 Caffeine: Caffeinated beverages like soda, coffee or tea, may have a mild diuretic effect, depending on the caffeine level.  Alcohol: It is also smart to avoid alcoholic beverages which have a diuretic effect, causing your body to lose water.

As a rule of thumb, be proactive about hydrating yourself to avoid the first and most common sign of dehydration: thirst. Drink before you venture out into the heat, during your activities (which includes sunbathing!) and after to constantly replenish your fluid stores.

2. Avoid courting heat illnesses.

Heat illnesses such as heat exhaustion and life threatening heat stroke can develop well after becoming dehydrated and happens when the body’s cooling mechanism is overloaded and unable to regulate its temperature. Those most prone to heat illnesses are the elderly, children and infants, and people with high blood pressure.

How do I know if I’m developing a heat illness?

 Symptoms: cool, moist, pale or flushed skin, muscle cramps, heavy perspiring, sometimes vomiting, weaknesses and dizziness, dry mouth and a headache are all symptoms of heat exhaustion. The more serious heat stroke can occur within 15 minutes of the first symptoms, which include very high body temperature, hot, dry, red skin, no sweating, dizziness, confusion, hallucinations, or loss of consciousness.

What do I do if I’m developing a heat illness?

 Get out of the sun: Getting into a cool place, loosening clothing, drinking caffeine-free liquids, taking a cool shower, bath or sponge bath and resting can alleviate heat exhaustion but a heat stroke is a medical emergency. Call for medical assistance and try to cool down rapidly with cool water. Do not drink any fluids. Call a hospital emergency room for further instructions if medical assistance is delayed in responding.

3. Protecting your skin from the sun’s UV radiation:

By now, everyone knows that sunscreen is a must. Few people use enough sunscreen, and not all sunscreens are created equal. Some may actually increase cancer risks, and their effectiveness drops when under-applied, meaning that a sunscreen with an SPF label of 100 performs like one with a SPF of 3.2, and a SPF 20 rating drops down to a 2.3, etc. Also scientists say SPF claims above 50 aren’t reliably substantiated. Moreover, sunscreen products aren’t regulated, so some existing products overstate claims about performance and offer inadequate UVA protection.

Apply sunscreen about 30 minutes before going out into the sun, and then reapply every 2-3 hours - or sooner if sweating or if going in the water (even if it says it’s waterproof!).

 What to avoid and what to look for in a good sunscreen?

Avoid: Avoid ingredients such as Oxybenzone Vitamin A, which is a retinyl palmitate,(listed in 41 percent of sunscreens) added insect repellents, sprays, powders and SPF above 50+

Use: Products with zinc, titanium dioxide, avobenzone or Mexoryl SX in creams. Water-resident products with broad-spectrum protection are great for the beach, pool and exercise and look for SPF 30+

 What to do before applying sunscreen?

Avoid getting burned: Red, sore, blistered, then peeling skin is a clear sign of too much sun. Sunburn increases risk for skin cancer.

Use clothing: Shirts, hats, caps, shorts, pants, shield skin from the sun’s UV rays. Instead of coating the skin with goop, a long sleeve shirt is more effective.

Use shade: Take advantage of shade trees, umbrellas and canopies. Sunglasses are a must to protect eyes from UV radiation which causes cataracts.

Plan ahead: Go outside in the early morning or late afternoon when the sun is lower in the sky, rather than in the middle of the day when the UV peaks around midday and when the sun is directly overhead.

On October 4, Summit Medical Group (SMG) volunteers tied pink ribbons to trees at SMG's 1 Diamond Hill Road location in support of Breast Cancer Awareness Month, in an annual event that takes place at all nine of SMG’s New Jersey locations. Provided by the Susan G. Komen Foundation, the ribbons serve as reminders to women that early detection via mammogram is the best prevention against breast cancer.

According to the American Cancer Society (ACS), US breast cancer estimates for 2010 suggest that:
207,090 new cases of invasive breast cancer will be diagnosed in women 54,010 new cases of carcinoma in situ, a noninvasive and the earliest form of breast cancer, will be diagnosed 39,840 women will die from breast cancer 1 in 8 women will ever develop invasive breast cancer

“The good news is that death rates from breast cancer are declining, most likely as a result of increased awareness and screening, early detection, and more effective treatments,” says Carol Boyer, RN, MSN, APN, CBPN-C of SMG’s Breast Care Center. She adds, “Early cancer detection is key to maintaining breast health. We can help you stay on top of your health through screening, education, accurate diagnosis, and personalized treatment. Our certified breast health nurse navigators will partner closely with you to be sure you receive the care you need.”

Ms. Boyer suggests that women follow the ACS screening guidelines as well as conduct monthly self breast examinations. “In addition to clinical screenings and mammograms, taking time for self breast examinations is a way patients can work with practitioners to achieve their health goals.”

In accordance with the ACS, SMG recommends that patients:
Begin self-breast examinations at age 20 to become familiar with how their breasts look, feel, and change Have yearly mammograms beginning at age 40 and continue them yearly as long as you are in good health Have clinical breast examinations every 3 years between the ages of 20 and 30 years Have yearly clinical breast examinations from age 40 on Be screened with magnetic resonance imaging (MRI) in addition to mammograms if there is a family history of, genetic tendency toward, or other risks for breast cancer

Because each woman’s health and medical history are unique, screening needs may differ. Women should ask their doctor whether additional tests or screening at an earlier age is necessary.

Exercise May Reduce the Complications of Diabetes, According to Sports Health Journal Study

Activity may delay diabetes onset in those with glucose intolerance

Exercise is a critical piece of a healthy lifestyle, however, those who suffer from diabetes may see an even greater impact, according to a study published in the January/February 2010 issue of Sports Health: A Multidisciplinary Approach. Authors confirm that exercise can aid in diabetes treatment by improving glucose metabolism and insulin sensitivity.

When a healthy individual eats a meal, the pancreas produces insulin. This insulin helps the body store and use the glucose, or sugar, present in the food for energy and everyday activities. A body affected by diabetes either does not create enough insulin or does not properly use the insulin produced. When insulin release and glucose levels are not balanced, either high or low blood sugar can result.

This is where exercise can help. “During exercise, insulin is suppressed to allow more glucose to be released from the liver, but, it also makes the muscle cells more sensitive to insulin, allowing for a more efficient use of the glucose,” explains Karl B. Fields, MD, an author of the study and a physician at the Family Practice Center at Moses Cone Hospital.

There are two types of individuals affected by diabetes: Type 1 individuals are typically thin and are affected by diabetes at a young age. In these individuals diabetes is caused by an autoimmune process that destroys insulin-producing beta cells in the pancreas. Type 2 individuals are more commonly overweight adults, though the disease has become more prevalent in younger age groups due to obesity. Type 2 diabetes is caused by insulin resistance and relative insulin deficiency. The study notes that each group can benefit from exercise, however, it is important for a diabetic athlete to consult with a physician before beginning an exercise program to assess any needed adjustments to insulin intake.

“Diabetic athletes should meet with their physician regularly to assess glycemic control. Their physician will establish a baseline hemoglobin A1c. Type 1 diabetics with a baseline level higher than nine percent should not participate in vigorous exercise until the baseline control is improved. Type 2 who have a poorly controlled baseline levels should begin an exercise program with low intensity, increasing intensity as control levels are improved,” advises Fields. In individuals without diabetes, the baseline level is typically less than six percent.

The study provides some other recommendations for athletes with diabetes. These include:
 Aerobic exercise five days a week (a minimum of 150 minutes a week)  Resistance training three days a week  Consumption of a carbohydrate-rich meal before exercise (insulin-dependent diabetic)  Blood glucose monitoring before, during, and after exercise Diabetic individuals can also improve their health by smoking cessation, blood pressure control, and lipid management. “It is very important that an individual suffering from diabetes take progressive steps in creating a healthy lifestyle; changes to diet and physical routine can make a significant impact in diabetes treatment. But, in order for the changes to be beneficial, they have to be long-term,” Fields adds.
