VCU Medical Center and Richmond Ambulance Authority partner in unique effort

Virginia Commonwealth University Medical Center and the Richmond Ambulance Authority have improved resuscitation and survival rates dramatically for cardiac arrest patients by training and equipping paramedics to begin lowering a patient's body temperature in the field during resuscitation and following up at the hospital with a host of high-tech strategies to improve the odds of survival.

The VCU and RAA initiative, known as the Advanced Resuscitation Cooling Therapeutics and Intensive Care Center, or ARCTIC, is the most comprehensive program of its kind in the United States, and its strategy resulted in an almost two-fold improvement in the return of spontaneous circulation, from 25 percent in 2001 using conventional treatments to 46 percent in 2008. In turn, the survival rate to hospital discharge improved from 9.7 percent in 2003 to 17.9 percent at the end of 2008. The national average is less than 7 percent.

ARCTIC has two goals: to restart the heart as quickly as possible following onset of cardiac arrest, and to protect the brain by starting cooling as early as possible and bringing resuscitated patients to a single specialized post-resuscitation facility. Between 2001 and 2008, the team evaluated 1,598 cases of adult, out-of-hospital cardiac arrest events in Richmond, Va., and concluded that a building block strategy comprised of a unique combination of mechanical chest compressions, airway management, drugs that restart the heart, and cold saline given during resuscitation prior to the return of spontaneous circulation, sequentially improved patient outcomes.

"What we now know is that we have to protect the brain and vital organs during resuscitation and after the heart is restarted and this has led to a totally new strategy for how we treat cardiac arrest patients," said Joseph P. Ornato, M. D. (http://www. vcu. edu/mcved/faculty1.html), chair in the Department of Emergency Medicine (http://www. vcu. edu/mcved/welcome1.html) at the VCU School of Medicine and operational medical director of the Richmond Ambulance Authority (http://www. raaems. org/).

"Richmond Ambulance Authority paramedics are the first in the country to initiate the cooling process during resuscitation. RAA's treatment strategy includes using a combination of drugs given early to support circulation and restart the heart, and performing high quality CPR using a mechanical device - available in all RAA ambulances - that squeezes the chest to achieve better blood pressure and oxygen delivery than that achieved with just standard CPR," he said.

"The Richmond Ambulance - VCU Medical Center partnership is so successful because it provides a continuum of care for the cardiac arrest patient from the time the 911 call is received until the patient walks out of the hospital. We are constantly sharing and assessing detailed data to improve the clinical and operational process, and the dramatic increase in our survival rate reflects this," said Chip Decker, chief executive officer of the Richmond Ambulance Authority.

Decker continued, "The EMS system in the city of Richmond is a team approach between the Richmond Ambulance Authority and Richmond Fire Department, with Fire as first responder, and it works extremely well. Our rapid response times, combined with leading-edge clinical protocols like the cooling process implemented under Dr. Ornato's guidance, have enabled us to deliver a more viable patient to VCU."

VCU Medical Center is continuously staffed with a team of specially trained physicians and nurses experienced in post-resuscitation care who continue the rapid cooling process by placing a high-tech plastic coil into a large vein soon after arrival at the emergency department. Patients are treated by specially trained coronary care unit nurses and physicians who administer complex "goal-directed" treatment protocols and maintain the patient's body temperature at 93° F for at least 24 hours, following which the patient is gradually rewarmed in a computer-controlled sequence.

In previous work, researchers have found that patients who undergo controlled hypothermia using simple techniques such as cooling blankets and ice packs, have a better chance of brain recovery and survival following cardiac arrest than those whose body temperature is not lowered. However, such techniques are crude and often result in large temperature swings. The catheter technique used at VCU allows precise control of the cooling and rewarming process in an attempt to minimize brain injury following the cardiac arrest. The comprehensive ARCTIC approach is showing greater benefit than that which was seen using just conventional resuscitation and simple cooling techniques alone.

"One of the novel things about VCU's ARCTIC program is that we have been able to seamlessly incorporate the care of the cardiac arrest patient from the time they arrest in their home to the time they return home with good neurologic survival," said Mary Ann Peberdy, M. D. (http://www. pauleyheart. vcu. edu/faculty_peberdy. htm), Professor of Medicine and Emergency Medicine in the Division of Cardiology at the VCU School of Medicine.

"We have been able to develop a partnership with our EMS system as well as our referring hospitals and initiate therapies that are unparalleled in the country. The care that this multidisciplinary team provides gives patients better outcomes than we have seen with traditional cooling alone," she said.

The team presented their findings on Nov. 14 and 15 during the Resuscitation Science Symposium at the American Heart Association Meeting in Orlando, Fla.

In a second study of 181 consecutive, out-of-hospital cardiac arrest patients who were successfully resuscitated in the field and brought to VCU from various EMS agencies or other hospitals throughout Central Virginia between 2001 and 2009, the study showed survival to hospital discharge was 19% in 2001-3 when standard post-resuscitation care was provided. It increased to 38% in 2004-7 when simple cooling techniques were added, but further improved to 49% in 2008-9 using the full ARCTIC strategy.

Additionally, patients whose cardiac arrest was caused by the abnormal rhythm, ventricular fibrillation (VF), now have a 72% chance of surviving to hospital discharge if treated in the ARCTIC program. The aggressive ARCTIC treatment also improved the neurological outcomes for VF patients over simple cooling alone.

Additional Background

Cardiac arrest is a condition in which the heart suddenly stops beating, most commonly due to a rhythm disturbance known as ventricular fibrillation (VF). Within 10-20 seconds, the brain and other vital organs are deprived of oxygen, and the victim collapses to the ground unconscious and pulseless. Delivering an electrical shock to the heart promptly with a defibrillator can often start it beating again, but a defibrillator is not often available immediately. Cardiopulmonary resuscitation (CPR) can provide life sustaining blood flow and oxygen delivery to vital organs until trained rescuers can arrive and administer standard resuscitation techniques such as defibrillation, drug therapy, and other treatments. Unfortunately, survival from out-of-hospital cardiac arrest averages only 6-7%, claiming the lives of approximately 350,000 Americans each year using standard techniques. In many communities, paramedics can initially resuscitate cardiac arrest victims, but the majority will never regain consciousness and die due to brain injury.

It has long been thought that the brain can only survive without blood and oxygen for 4-6 minutes before irreversible damage occurs. That is now known to be incorrect. Brain cells begin to show signs of abnormal function soon after they are deprived of oxygen, but recent studies show that permanent damage begins after the heart restarts and oxygen returns to the brain. The cells try to consume the oxygen, but their injured machinery converts the oxygen into poisonous chemicals called "free radicals" which kill the cells. Cooling the brain as quickly as possible, ideally before the heart even restarts, slows its metabolism so it can't generate a large quantity of "free radicals". Maintaining precise temperature control for 24 hours and rewarming gradually using the catheter device is designed to prevent large swings in the body temperature which can result in a surge of metabolism and "free radical" production.

Expanding Emerson's DS line of distributed power solutions, the new DS2900 is ideal for use in high availability telecommunications applications and a variety of enterprise networking environments, including server farms and large storage systems.

The Emerson DS2900 is rated for up to 2,900 W of adjustable output power at 240 A. In addition to the 12 Vdc output, it features as standard a 3.3 Vdc auxiliary output for powering standby circuitry and an optional alternative 5.5 Vdc auxiliary output. The compact 2U x 3U form factor of the DS2900 offers customers a high system-level power density of more than 24 W per cubic inch. As with many Emerson power supply solutions, the DS2900 features market-leading typical operating efficiency of approximately 91 percent at 50 percent load. The power supply's shorter form factor also enables mid-plane rack configurations, while same-side ac inputs and dc outputs allow hot-plug rack configuration.

The power supply accepts a wide input voltage range of 180 Vac to 264 Vac with no minimum load requirement and is equipped with active Power Factor Correction (PFC) rated at 0.99 typical to minimize input harmonic current distortion. It also features active ac inrush controls - limiting inrush current at power-on to 55 A - and is protected against overvoltage conditions up to 130 percent.

The DS2900 is equipped with a compact (40 mm by 28 mm), variable speed "smart" fan supported by integrated controls to enhance the product's energy efficiency and reliability. To minimize unplanned downtime and enhance serviceability, the DS2900 features both N+1 redundancy and internal ORing controllers. The DS2900 supports a wide operating temperature range of zero to plus 50 degrees Celsius and when used under normal operating conditions (25 degrees Celsius ambient at full load), it offers a high demonstrated mean time between failures (MTBF) of more than 500,000 hours.

To provide customers with real-time test, monitoring and configuration functionality, the power supply is digitally programmable and fully compatible with Emerson Network Power's PMBusTM graphical user interface software via an I2C communication interface bus.

The power supply complies with EN55022 and FCC part 15 Level "A" EMC standards for conducted noise, including all applicable immunity standards. It also carries UL/cUL60950 safety approvals and is compliant with Restriction of Hazardous Substances (RoHS) directives.

With support from coworkers, family and friends, TRI-AD Associate Irma Lopez walked 60 miles to support breast cancer research in the Breast Cancer 3-Day, benefiting Susan G. Komen for the Cure and the National Philanthropic Trust Breast Cancer Fund. Part of a series of events in 12 cities, San Diego's 3-Day Walk was held November 21 - 23, 2008 and raised an estimated $11 million.

Ms. Lopez joined 5,550 other participants who walked about 20 miles each day of the event. Registrants were required to raise at least $2,200 in donations to participate. Through an array of fundraising activities, including silent auctions, for-donation potluck lunches, a "penny war," craft and bake sales, TRI-AD's Associates helped Ms. Lopez meet her fundraising goal. "I just couldn't believe how much everyone got into helping me meet my donation goal. It seemed like everyone here had a relative or friend who had been affected by breast cancer. I walked for all of them."

Ms. Lopez wore a special sash made for her by fellow TRI-AD Associate Ann Beckett. Adhered to the pink satin sash were 60 pink ribbons, each bearing the name of a woman affected by breast cancer, including a special ribbon and photo of Ms. Lopez' mother-in-law, Rosa Serrano, who succumbed to the disease in 2004. Some ribbons represented survivors, others were in memory of women who lost their battle against the cancer that will claim the lives of 40,000 people this year.

TRI-AD Vice President Thad Hamilton praised TRI-AD's Associates for their fundraising activities and for supporting Ms. Lopez in her endeavor. "Surprising Irma with a 'Pink Potluck' the day before the walk capped off almost a year's worth of fun and meaningful ways to show support for Irma and help her reach her goal. We are all proud of Irma for joining this challenging event and for raising awareness here about the fight against breast cancer."

They took an enriched cell culture medium that is commonly used to grow skin cells in petrie dishes and applied it directly to wounds and watched to see what would happen. “It was remarkable and almost immediate,” recalls Dr. Lindenbaum. “We saw wounds that resisted all conventional approaches begin to heal dramatically.” This confirmed to the team of researchers what they already guessed; this very potent and natural technique had extraordinary healing power that ultimately proved its ability by closing over two thirds of all the resistant wounds.

That would have been enough for any research team to be completely satisfied but what happened next was remarkable.. In 1998 a patient was brought to the University to receive therapy. It was the teen age daughter of the President of Uzbeckistan. The High Capacity Line of re-programmable memory devices includes two families of memory tokens: the DFX Series, which delivers a high-performance Secure Digital (SD) interface (including SPI); and the UFX Series, which delivers a USB 2.0 High-Speed interface. Both series come standard with 4GBytes of non-volatile memory, with larger capacities (up to 32GBytes) available. With SPI, SD and USB interface options, these new memory tokens are easily integrated into embedded controller, single board computer, and industrial PC designs, as many of these devices come standard with one or more of these serial interfaces. Utilizing Datakey Electronics’ robust SlimLine™ token form-factor design allows the DFX and UFX tokens to leverage the entire line of SlimLine™ receptacles, providing OEMs with a complete data-carrier system. The High Capacity Line of receptacles features multiple mounting options including through-hole, surface-mount, and panel-mount versions. Panel-mount options are available in IP65 (“splash-proof”), IP67 (immersion), and EMI reduction versions to meet the needs of harsh environment applications. All of the receptacles are rated for 50,000 cycles and withstand shock & vibration, extreme temperatures, humidity, salt-fog environments and exposure to various chemicals.

The High Capacity Line's DFX and UFX rugged memory tokens differ from SD cards and USB flash drives in three important ways:

 The memory tokens utilize Datakey Electronics' proven SlimLine™ contact system. Their mating receptacles have a high cycle-life rating of 50,000 cycles, compared to just 1,500 cycles for most USB Type A receptacles. At just 10-cycles per day, a USB Type A receptacle will hit its rated cycle-life in only five months. At the same 10-cycle per day rate, the SlimLine™ receptacle won't hit its rated cycle-life for more than 13-years!  The memory token's unique form-factor means that it can't plug in to "just any" PC, which helps to protect the data on the token. The unique form-factor can also prevent consumer devices from plugging into the OEM system, which helps protect that system from unauthorized access, and prevents malicious files (like viruses) from being uploaded. The token's controlled availability virtually assures that only approved devices are used in the OEM system and provides the OEM with an additional revenue stream from the sale of memory tokens to their users.

America's leading organization for the promotion and advocacy of flossing -- declares the theme for this year's National Flossing Day (NFD) to be "Floss With A Friend." As such, the Council is encouraging people across the country to take the day after Thanksgiving -- Friday, November 27th, 2009 -- to engage in interpersonal flossing activities.

Making today's announcement was National Flossing Council President Armand Lione, who said: "The Thanksgiving holiday weekend is known for friends, family and food. Now is the time to add a fourth "f" -- Floss. We recognize that with flossing, as with most things, the more the merrier. Therefore, we declare this year's theme 'Floss with a Friend.' To help people get into the spirit of the event, we have posted flossing games on our website, www. floss. org. Your family and friends can enjoy each other's company, better oral hygiene and better health -- while flossing your teeth."

The flossing activities range from classic string games, like "Cat's Cradle." to newer games that use floss to promote a healthy body image.

During the last decade, the focus of NFD has ranged from Floss/Food Art parties to the presentation of our media award, The Flossy, to Ellen DeGeneres, who has done a lot to promote flossing! For NFD 2007, the Flossing Council requested that McDonald's restaurants begin offering individual packs of floss with each of its meals. National Flossing Day 2008 focused on flossing etiquette by offering downloadable signs to make clear where flossing is ok at home and in the office.
