The secret to losing weight and keep up with a hectic schedule is to change your attitude and behaviors toward food.

So you work a fourteen-hour day, travel around the world, and have little time to exercise. You now complain you can’t fit into your wardrobe. “How did I gain those twenty-five pounds?”

Well, if you had the time, a food record would show you the answer. You would see the following: 1) Much less leisure activity - I just donÂ’t have time for the gym

2) No real time schedule to your meals. It is 3 a. m. London time!

3) High junk intake at airports and on planes. I really hate airplane food.

So what is the secret to lose weight and yet keep a hectic schedule? It is a combination of attitude and behavioral changes that you will need to take. For this to really work over the long-haul, you need to make permanent changes. As I say in my book, The Thinderella Syndrome, “You must live a diet and not just do a diet!” Let’s look at a typical (if there is such a thing) working day. If you have a routine schedule and it repeats itself daily through the week, you can pre-plan many of your eating events. Treat eating as a client. Schedule eating into your planner. Breakfast, lunch, or snack can be put in based on time of hunger, not time of the day. I typically have a fruit at seven a. m. in the office and have lunch at eleven a. m. I don’t force myself to eat at twelve to one p. m., so don’t go by the clock.

Strict Avoidance - “Out of sight, out of mouth.” For the busy entrepreneur, this requires a bit of pre-planning. The basic premise here is to protect yourself from the (obesifying effect of the environment). To build this protective fence requires that you have a healthy, easy to get to food that is transportable and readily available. I typically have a bag of fresh fruits packed the night before that I just bring into the office. I make sure not to order foods in or go to areas of the office where snacks are kept.

During travel time, fruit is convenient and clearly a healthy substitute to airport foods. Other pre-planning ideas include packing a healthy lunch in an “igloo” lunchbox, thus avoiding ordering in more fattening foods.

Those of us who meet in restaurants “business meetings” have a harder time. Here the technique of “Strict Avoidance” is more difficult, but don’t give up hope! You can frequent restaurants that cater to low-calorie foods. Some of my business clients have their favorite restaurants catered to their needs by serving them the “usual” salad or low-cal dish, even if it is not on the typical menu. As a general rule, I would tell the frequent restaurant visitor to treat the restaurant as if it is your own kitchen. Eat healthy meals that are small in portions and choose a weekend meal or special event to have a regular restaurant meal with dessert and all.

“Other tips” (pardon the pun) on Restaurant eating:

• Don’t have breadbaskets in front of you. Send them back or move them away from you.

• Drink water or seltzer and avoid the caloric alcohol beverages.

• Pre-plan the meal you want so you can order without being sabotaged by the menus.

• Try to avoid restaurant meetings, look at other social arenas for business get-togethers such as coffee cafes, a golf game, a squash game, or any non-food entities.

• When you must order in the office for business meetings, be sure to have plenty of salads and fresh fruit available even if you feel obliged to serve your client more enticing items.

Traveling poses another challenge since many times you donÂ’t know what or even where the facility may be.

Some general points:

• Avoid room service and buffets, they are always a challenge.

• Fruits and veggies will generally be available if you are assertive and ask for them.

• Some entrepreneurs like to carry “burn bars” or other health bars with them. Ultrameal bars are zone-based and nutritious. They are 150 to 200 calories per bar and certainly better than most travelers’ foods.

Exercise is another challenge. Too often the complaint is, “I am exhausted, I work eighteen hours a day, I can’t squeeze it in.”

• Remember that there are many forms of exercise, even if you can’t go to the local gym, try getting out of the cab and walking a few blocks.

• Walk up a few stairs instead of using the elevator. Don’t look for the closest parking spot when you go to a meeting. These minimal tasks over time consume energy and will burn calories.

• Get a pedometer to check out how much physical activity “in everyday lifestyle” you get.

DonÂ’t be frightened by exercise, you can get leisure activity in. If you feel you need to get things done while you are exercising, here are some ideas:

• For reading, try the stationary bike. You can read and write reports while pedaling away.

• If you want to return phone calls, do it on the move, and use your cellular phone while walking.

• Need to follow the business news, watch your business channel while walking or jogging on a treadmill.

• Finally, look at exercise as an enjoyable break from a grueling work schedule. You deserve a breath of fresh air.

In an era dominated by industry-supported medical research, physicians are forced to carefully examine their treatment decisions to ensure that they are truly acting in patients' best interests. Today at the 33rd annual VEITHsymposium™, distinguished vascular surgeons explored the ethical pressures doctors face as a result of the rapid rise of industry-driven medical research, and the best tools available to them to help ensure objective decision-making.

Dr. Timothy M. Sullivan, a Professor of Surgery at the Mayo Medical School and a Mayo Clinic Consultant (Rochester, MN), spoke eloquently about dramatic changes to the physician-medical device industry relationship in recent years. Previously, few products existed, so industry had little part in surgeons' decision-making. Today, the opportunity to become involved in high-profile device trials is extremely desirable to vascular surgeons and their practices. The balance of the physician-industry relationship has shifted in favor of device manufacturers, who often seek influence over the design, conduct, analysis and publication of trial results, sometimes discouraging physicians from publishing negative findings. Physicians also increasingly rely on industry for training instead of academia, developing a dependence that colors their ability to view new technology with objectivity. "As physicians become more involved with industry and serve on Boards of Directors, they face both fiduciary responsibilities to investors and a responsibility to remain neutral regarding outcomes for the good of patients; these two responsibilities cannot be reconciled," said Dr. Sullivan.

How can physicians resolve these ethical and moral dilemmas? Dr. Thomas E. Brothers, Program Director in Surgery at the Medical University of South Carolina (Charleston, SC), points vascular surgeons to the use of Markov models, which can help them rely less on subjective decision-making and more on the objective analysis of data. A Markov model enables the analysis of complex problems by finding "hidden" data in variables that can be readily observed. In a recent trial of 206 patients with symptomatic lower extremity arterial disease, Markov model estimates of treatment outcome probabilities were compared to actual treatment plans provided by vascular surgeons.

The study found that the Markov model treatment plans offered significantly better quality of life outcomes for patients in the most severe pain and among those undergoing bypass. In addition, patients for whom the model disagreed with the surgeon's initial treatment plan were 17% more likely to undergo bypass, and were generally less likely to report a favorable quality of life one year later.

"Our findings confirm the complexity of surgical decision-making for patients with symptoms of chronic lower extremity arterial occlusive disease. Routinely treating all lesions that are discovered, as increasingly seems to be advocated by some cardiovascular specialists who have recently included peripheral intervention to their scope of practice, without a profound understanding of the implications of their actions in view of the natural history of the disease, cannot be supported. As has always been the case, restraint and careful case selection with the patients' best interests in mind must remain the hallmark of the vascular specialist."

Physicians seeking the best way to "do no harm" in the complicated environment of modern industry-driven medicine may increasingly turn to objective measures like the Markov model in order to provide the best possible treatment for patients.

Partnering with community-based agencies throughout Haiti and the Dominican Republic, AJWS supports grassroots human rights and community development projects that are designed, implemented and managed by the organized poor.

Since late August 2008, heavy wind and rain, floods and mudslides in Haiti (http://en. wikipedia. org/wiki/Haiti) and the Dominican Republic (http://en. wikipedia. org/wiki/Dominican_Republic) have killed approximately 1,500 people and have cut off ground access to 40,000 people due to washed out bridges and roads. While many still have not recovered from last year's extremely active hurricane season, continued storms have left close to one million homeless.

The hurricanes have had a significant impact on Haitian farmers and rural communities where more than 50,000 families have lost access to their agricultural production. The people that are predominately affected by the storms in the Dominican Republic include the Haitian and Haitian-Dominican population, a community already grappling with disproportionate poverty and a lack of infrastructure. Haitian and Haitian-Dominicans are excluded from government sponsored initiatives and services because they are not legally recognized or considered a minority or ethnic group.

-- Programs funded by AJWS in Haiti provide access to new technology and training to enable agricultural cooperatives; technical support and resources to strengthen peasant organizations addressing agricultural productivity and food security; and health literacy and business skills training to clients of village banks.

-- Programs funded by AJWS in the Dominican Republic are driven by the priorities of Haitian-Dominican communities struggling to secure basic civil and political rights and legal protections. AJWS helps Haitian-Dominican organizations to integrate health services--including HIV prevention, testing and treatment--into their ongoing human rights work. Increasingly AJWS is in a position to foster strategic alliances between its grantees and other international development agencies for networking, service referrals and learning opportunities.

Regarding the recent hurricanes, AJWS is raising awareness and collecting funds in the Jewish community for emergency response efforts undertaken by its grantees. AJWS is partnering with two grantees in Haiti to provide agricultural inputs and infrastructure reconstruction support to women and farming families most affected by the flooding. AJWS emergency grants will not only meet some of the immediate needs of the affected communities but will also advance their long-term development goals, especially as they relate to reducing disaster vulnerability.

VivaTRAK is a complete system that monitors the activity of patients and their caregivers in nursing homes. The system ensures that care tasks are completed on time and documented in healthcare records for quality assurance, reimbursement and liability-risk mitigation purposes. It is a patient-focused and sensor-assisted care system that uses a patented fiber optic sensor to monitor a patient’s activity while in bed, processes this data in a proprietary data server, provides the appropriate notifications, and reports to the nursing staff – and then verifies their response to record that care was actually delivered.

The 510(k) included data from a successful clinical evaluation at the Raleigh Court Healthcare Center operated by Medical Facilities of America, Inc. (MFA). The clinical evaluation was conducted with oversight by the Institutional Review Board of the Carilion Clinic located in Roanoke, Va.
