Taxpayers Say More Relief Is Needed; Corporations, Not Individuals, Should Be Top Tax Revenue Source, Finds CCH Survey

Taxpayers say proposed tax legislation does not go far enough; want more tax breaks; and express their opinions on where government income should come from and how to spend it in CCH CompleteTax nationwide survey.

Taxpayers believe more needs to be done to help them than the tax legislation currently being debated in Congress, according to the nationwide CCH CompleteTax (http://www. completetax. com/) survey of nearly 900 taxpayers.

The survey, commissioned by CCH and conducted by GfK Roper, also asked taxpayers about their opinions on where the government revenue should come from and what should be priorities for government spending. Survey findings reveal taxpayers' views are not the same as actual government action.

"This tax season people are confronted with a variety of tax issues from the immediate need to complete their own tax returns to the larger debate about broad tax policy moving forward," said David Bergstein, CPA, a tax analyst for CCH CompleteTax®.

"All of this is amplified by a weak economy that has battered taxpayers' wallets over the past year. They're clearly saying they need tax breaks and they want change in Washington," added Bergstein.

More is Needed

The CCH CompleteTax survey asked taxpayers which of the various tax-break provisions being debated in Congress as part of the tax stimulus package (http://tax. cchgroup. com/Legislation/2009-Recovery-Act. pdf) would help them the most.

Nearly two-thirds of taxpayers (63%) said all the provisions identified would help, but that more needed to be done. Among the individual provisions cited as the single most helpful to taxpayers:
 A credit of up to $500 against a taxpayer's income tax, according to 9% of taxpayers overall; taxpayers earning $30,000 or more selected this tax break more often than other taxpayers as the single most helpful;  Increasing the amount of Earned Income Tax Credit (EITC) available to low-income taxpayers, according to 9% of taxpayers overall; this was the single most cited tax provision identified by taxpayers earning less than $30,000 as most helpful;  An increase in the Hope higher education credit from $1,800 to $2,500, according to 7% of taxpayers;  Making the $1,000 child tax credit available to more taxpayers with dependents, according to 7% of taxpayers; and  A credit to first-time homebuyers that does not require future repayment, according to 5% of taxpayers. "Many in Congress also believe more needs to be done, which is why more proposed tax breaks were added to the legislation on the Senate floor," said Bergstein.

Saving for Retirement Tops Tax Breaks People Want

With many people's 401(k) and other retirement savings severely diminished after last year's poor stock market performance, taxpayers overwhelmingly want to see tax breaks for fortifying their nest eggs. According to CCH CompleteTax survey respondents, the biggest tax breaks for taxpayers should come from:
 Saving for retirement, identified by 84% of taxpayers; individuals 35 years old and older selected this more often than younger taxpayers;  Buying and owning a home, identified by 82% of taxpayers;  Saving for education, identified by 79% of taxpayers; young taxpayers, under age 35, selected this more often than older taxpayers;  Having and raising children, identified by 77% of taxpayers;  Donating to charities, identified by 70% of taxpayers; and  Living in an environmentally friendly way, identified by 69% of taxpayers.

President for a Day: Taxpayers Prioritize Government Income and Spending

Each year, the federal government reports its sources of income and outlays. Among its top 2007 income sources, the most recent available, were:

1. Personal income taxes, accounting for 43% of income;
2. Social Security, Medicare, unemployment and other retirement taxes, accounting for 32% of income;
3. Corporate income taxes, accounting for 13% of income; and
4. Excise, customs, estates and gift taxes, accounting for 6% of income.

However, if taxpayers had their way, they would reorder revenue sources. According to the CCH CompleteTax survey, taxpayers believe corporate income taxes should be the largest source of federal income followed by personal income taxes and excise, customs, estate and gift taxes.

As for spending, taxpayers' priorities also don't appear to be the same as actual federal outlays. For 2007, federal government spending was on:

1. Social Security, Medicare and other retirement programs, 38% of spending;
2. National defense, veterans and foreign affairs, 23% of spending;
3. Social programs, including Medicaid, food stamps, unemployment compensation and public health programs, 19% of spending; and
4. Physical, human, and community development, including education, environment, transportation, job training and housing credits, 9% of spending.

If taxpayers were in charge of the federal checkbook, however, their priorities would be different. According to the CCH CompleteTax survey, taxpayers believe the most should be spent on physical, human and community development. Following this, taxpayers would spend most on Social Security, Medicare and other retirement programs, followed by national defense, veterans and foreign affairs.

"Taxpayers may be choosing development as their top choice because they see it as the area of spending most likely to help ensure job creation and help restore the economy," said Bergstein.

Survey Methodology

The nationwide telephone survey was conducted by GfK Roper on behalf of CCH CompleteTax from January 30-February 1, 2009, among 871 taxpayers (age 18 and over). The margin of error on weighted data is +/- 3.5 percentage points.

When Daniel Hines launched www. “Whereas the older elderly population had little association with computers and the Internet, the Baby Boomers are highly familiar—and comfortable with—the Internet as a part of their daily lives,” Hines continues.

Hines points out that Boomers, defined as those born between 1946 and 1964, represent the largest demographic group in the U. S. at over 77 million strong, or about 27 percent of the population, according to the US Census Bureau.

“Living in an Aging America was the subject of a recent seminar for media conducted by The Century Foundation, which had as a goal making media increasingly aware of the impact that a growing number of older persons will have on our society.

“We believe that this aging population can be empowered through the Internet by becoming increasingly informed on the impact of issues over a broad spectrum that affect them.”

He says a major emphasis will be the ingredients of successful aging that will enhance the well-being of an aging population—fitness, health care, delivery of medical services, relationships, financial planning and continuing to work or live in a fashion that contributes to defining a new type of senior—one for whom aging is an enjoyable, important process of a successful life.

“I am confident that this century will witness the emergence of a better informed, healthier, happier older population than any time before,” Hines says. “The Internet will make a major contribution to attaining this goal.”

One-Dose Radiation Found Effective For Early Breast Cancer

A single dose of radiation administered during surgery is as effective for patients with early forms of breast cancer as standard radiation therapy that can take as long as six weeks, according to new research findings.

San Francisco, CA (Vocus) June 9, 2010

A single dose of radiation administered during surgery is as effective for patients with early forms of breast cancer as standard radiation therapy that can take as long as six weeks, according to new research findings.

A 10-year, international multicenter clinical trial found single-dose intraoperative radiotherapy was both safe and effective, with results comparable to conventional external beam radiation therapy delivered daily over the course of weeks. 

The findings could have significant implications in both time and expense for patients – particularly those with early forms of breast cancer – as well as the healthcare system because the patient receives all required radiation before awakening from surgery, said the trial leaders.

UCSF, which enrolled nearly 100 patients in the trial, was the lead cancer center in the U. S. Altogether the randomized controlled trial involved 2,232 breast cancer patients with invasive ductal carcinoma who were treated at 28 centers in nine countries throughout Europe, North America and Asia.

Preliminary results were presented today during the 46th annual meeting of the American Society of Clinical Oncology in Chicago. The June 4-8 meeting is expected to draw some 30,000 cancer specialists from around the world.

Long term follow up of the clinical trial will be continuing.

“Radiation therapy for women who have had a lumpectomy typically lasts three to six weeks, Monday through Friday,’’ said UCSF Principal Investigator Michael D.
Alvarado, MD, a breast cancer surgeon at the Carol Franc Buck Breast Care Center at the UCSF Helen Diller Family Comprehensive Cancer Center. “This can be a significant burden for women across all socioeconomic levels. It can be especially difficult for women who need to travel a significant distance on a daily basis. By giving radiation as a one-time dose during the lumpectomy, the woman does not need to come back for radiation treatments.

“What is exciting and new is that the radiation is actually delivered during surgery. The radiation given in the operation room treats the area more precisely and the fact that it is immediate may also be beneficial,’’ said Alvarado who serves on the international writing committee of the project.

“The benefits are not only convenience; we believe it will be less dangerous than irradiating the whole breast because there is less ‘scatter’ to the lungs and heart, and fewer problems with the breast becoming hard after radiation.’’

During the last half-century, treatment of breast cancer has shifted from classical radical mastectomy to local treatment that preserves the breast and axillary lymph nodes along with adjuvant systemic therapy. Breast conserving surgery followed by postoperative whole breast external beam radiotherapy has become the standard of care for many patients with early breast cancer. While radiotherapy is safe and effective and the risk of side effects is low, the duration of daily treatments can be inconvenient or untenable.

The international clinical trial, called TARGIT-A (Targeted Intra-operative radiation therapy), was designed to determine whether single dose intraoperative radiotherapy is equivalent to standard external beam radiotherapy, which uses linear accelerators to irradiate the entire breast externally during a period that typically extends from three to six weeks.

In the trial, the one-time dose was administered in the operating room immediately after the tumor was removed. A miniature electron beam-driven device, INTRABEAM, manufactured by Carl Zeiss Meditec, was surgically positioned in the tumor bed and radiation was switched on for 20-35 minutes to target tissues at highest risk of local recurrence.

The median age of UCSF patients who underwent the treatment was 58.

“We are now trying to get a U. S.-based trial off the ground with major academic institutions across the country,’’ said Alvarado. “We are very excited about moving forward with this new technology.’’

Co-author Laura Esserman, MD, MBA, of UCSF and a member of the international steering committee for the trial, said “The target device is truly an innovation in radiation oncology and helps change our thinking about how to achieve breast conservation. For many women, the radiation therapy can be completed in one visit, instead of visits over a few weeks.’’

“This device truly revolutionizes patient management and the patient experience,’’ said Esserman, director of the UCSF Carol Franc Buck Breast Care Center and co-leader of the breast oncology program. “For patients this is amazing. We have identified a subset of patients for whom it is safe, and we should ensure that all such patients can access this technology. We will continue to study this device in higher risk patients to see if its role can be expanded.’’

Presenting the results during the ASCO meeting was lead author and co-principal investigator Michael Baum, MD, professor emeritus of surgery and visiting professor of medical humanities at University College London. He pioneered the targeted approach along with Jayant S. Vaidya, MD, PhD, consultant surgeon at the Whittington, Royal Free and University College London Hospitals; and Jeffrey S. Tobias, MD, PhD, of the Department of Radiation Oncology, University College London.

The trial “is the longest-running breast cancer study of its kind and it is also the first randomized partial breast irradiation study to be reported,’’ said Baum. “These study results, representing a decade of interdisciplinary clinical experience by leading surgeons, radiation-oncologists, scientists and physicists, has the potential of changing the way we think about the primary management of early breast cancer as well as financial considerations that impact the health care system.’’

Alvarado and Esserman said the single-dose therapy will be offered in the future in conjunction with the ATHENA Breast Health Network, a University of California statewide collaboration for breast cancer patients that designs and tests new approaches to research, technology and health care delivery. Under ATHENA, which launched in 2009, some 150,000 women throughout California are being screened for breast cancer and followed for decades through the five UC medical centers.
