The program covers drugs which by the federal law require a doctorÂ’s prescription and is covered by your medical plan. Check the drug formulary to see which drugs are available through the mail service. Unicare sends you a Certificate of Coverage that details your copayment, maximum days supply, deductibles and specific drug coverage and exclusions. Note: Insulin and Insulin syringes do require a prescription when ordered through the Unicare Mail Service Program.
You are eligible to enroll in the home delivery program if you and your family are currently covered by the outpatient prescription drug program through the pharmacy. When you order the first time you will need to include the form for enrollment of new participants and confidential patient profile forms. When you are placing your first order be sure to allow a few extra days for it to arive. Call for any further information toll free 1-866-526-9669.
